Selform MIS in an online portal developed by the President’s Office – Regional Administration and Local Government (TAMISEMI) for the selection process for secondary school (Form Five) and colleges in Tanzania. President’s Office, Regional Administration and Local Governments (TAMISEMI) is respionsible for proccessing selection for students to join colleges and high school for government schools and institutions based on performance of CSEE exam.

How to Change Combination for Form Five via Selform MIS Tamisemi 2025/2026

Students (Form Four Graduates) role in the Selform system is to change some personal information such as Home Address, Mobile Number, Email Address and school, college and specialty choices in other sections based on his performance in CSEE results.

Step 1: Use any device with internet access, open your browser (browser example, chrome), type the following address selform.tamisemi.go.tz to get a window to fill in your registration information as shown below. Click on the bottom Menu written

Step 2: For Candidates, Click here to Register if you are a first time user.

Step 3: Then, fill in the relevant information eg. Index Number, in this format example: S0101.0020.2018, Answer the question you will be asked, Surname and Year of Birth.

Step 3: Then a window will open, asking you to enter the Password that you will use to login every day.

Step 4: Once you have entered the Password, the system will appear in this form to show that you have successfully changed the Password.
